Teenage Booster (Diphtheria/Tetanus/Polio and Meningitis ACWY)
Just a reminder for parents of Year 9 Boys and Girls that the vaccinations are taking place here at school on Monday 14 January 2019 and Wednesday 16 January 2019. In order to be able to have the vaccination we must have a completed consent form (If you misplaced please ask your child to come and collect another and there will also be some in Main Reception).
The form should be completed and returned to the Attendance, Health and Wellness Officer via the Main Reception. If we do not receive a completed form then your child will be unable to be vaccinated. If you do NOT wish your child to be vaccinated the form does have a section which will need to be completed or alternatively you can email direct to firstname.lastname@example.org stating you do not wish your child to receive the vaccination
Please try to ensure your child is wearing a short sleeved blouse/shirt.
