This is vital to your peace of mind and your child’s well-being when in the event of an emergency we need to contact you urgently.

CHANGE IN MEDICAL INFORMATION

Has there been any change in your child’s medical information? For example, the following are all-important items to be noted on your child’s records:

  • newly diagnosed allergy
  • asthma
  • migraine
  • eczema
  • epilepsy
  • diabetes

This helps us to establish the severity of an illness when your child comes to the medical room.

However, should we have to accompany a child to hospital this information gives the nursing and medical staff a full picture when making a diagnosis.

Please complete and sign a Medicine Consent Form if your child needs prescribed medication.
